You have the possibility to complete the forms online: After validation of your entry, your dossier will be preregistered by our Services and the entered form will be sent to you via email to the email address that you provided. You can then print the form, have it signed by the authorised person, and address it to us by postal service (registered letter with return receipt) or by hand to our offices. If the paper version of the form is not returned to you within six months, your preregistration is cancelled. To start a preregistration online, click the following link:

If you want to modify an existing processing, you do not need to enter a new declaration online, please complete the sections of the form that you want to modify as well as the first and last pages of the form.
